Acknowledgment of Risk
Basketball training involves physical exertion and carries inherent risks, including but not limited to sprains, strains, broken bones, concussion, cardiac events, and other serious injury. I understand these risks exist in every session run by Rain City Athletics ("the Business"), whether held indoors or outdoors, and that no amount of supervision, instruction, or care can eliminate them.
Assumption of Risk
On behalf of myself and the minor athlete named below, I voluntarily accept and assume all risks of injury, illness, or loss arising from participation in training sessions, camps, clinics, and related activities, including travel to and from training locations.
Release of Liability
To the fullest extent permitted by law, I release, waive, and discharge Rain City Athletics, its owners, coaches, employees, contractors, and facility providers from any and all claims, demands, or causes of action arising out of or related to any loss, damage, injury, or illness sustained by me or the minor athlete while participating in Business activities, except where caused by gross negligence or willful misconduct.
Indemnification
I agree to indemnify and hold harmless Rain City Athletics from any claims brought by or on behalf of the minor athlete, or by third parties, arising from the athlete's participation in Business activities.
Medical Authorization
In the event of an injury or medical emergency, I authorize the coaches and staff of the Business to secure emergency medical treatment for the minor athlete if I cannot be reached. I accept financial responsibility for any such treatment. I confirm the athlete is physically able to participate in basketball training and I have disclosed any relevant medical conditions to the Business.
Photo & Media Release
I grant Rain City Athletics permission to use photographs or video of the athlete taken during sessions for coaching and promotional purposes (website and social media). No names or personal details will be published alongside images without separate consent. I may revoke this permission at any time by emailing the Business.
Agreement
I am the parent or legal guardian of the athlete named in this account. I have read this waiver and the Business Policy, I understand them, and I sign this agreement voluntarily. This agreement remains in effect for all sessions the athlete attends until revoked in writing.
